Certificate of Creditable Coverage

The Certificate of Creditable Coverage is a written statement used by a group health plan or health insurance issuer that shows prior health coverage (creditable coverage). The certificate must be issued automatically and free of charge when an individual loses coverage under a plan, when they are entitled to elect COBRA continuation coverage or when they lose COBRA continuation coverage. It must also be provided free of charge upon request while an individual has health insurance or within 24 months after their coverage ends. This will detail the dates of coverage and how long the coverage covered the individual. This is important for recipients to understand if their drug coverage is considered creditable under CMS rules.
