Activities of Daily Living

Activities of Daily Living, found in the function report SSA-3373, is used to evaluate how a claimant operates regarding daily tasks. The Activities of Daily Living form helps SSA determine if a person is eligible for benefits based upon their physical and mental limitations. The questionnaire covers a large range of topics including, but not limited to:

• Bathing and dressing
• Eating and drinking
• Using the bathroom
• Grooming (e.g., brushing teeth, shaving)
• Walking and standing
• Household chores (e.g., cooking, cleaning)
• Shopping and driving
• Social activities (e.g., visiting friends and family)
• Work and education

The SSA will use this as a function report to determine if an individual meets the requirements to be deemed disabled. It is important to fill out the form honestly and completely to give the SSA a clear picture of how a condition(s) affect an individual’s life.
